  1. Call on +91-98336-99033 / +91-81081-14337 and ask for an appointment with Doctor or Counesllor of your choice.

  2. Download Special History Form (Children or Adults as applicable) from here 

  3. Answer in details in another document as per guidelines.

  4. Email completed document to angels.appoinment@gmail.com or get a hard copy along when coming to the clinic. Hand written document will also do. 

  5. Get all medical reports and previous prescriptions along.

  1. Home visits are limited only within a limited distance within Mumbai and is offered to those who are unable to travel and need personal attention. It is decided as per doctor's discretion.

  2. Speak to the doctor before fixing an appointment. Call on +91-98336-99033 / +91-81081-14337 (no charges) .

  3. Once the visit is fixed, download the Special History Form (Children or Adults as applicable) and answer in detail as per the guidelines.

  4. Email completed document to angels.appoinment@gmail.com or keep a hard copy ready at time of Visit. Hand written document will also do.
